HealthBridge helps Medicare patients understand their health needs and coordinate care with clarity and respect. These Terms explain how the Services work and how we safeguard your information.

1) Acceptance of these Terms

By accessing or using HealthBridge services (the “Services”), you agree to these Terms of Service (“Terms”). If you do not agree, please do not use the Services.

If you use the Services on behalf of an organization, you represent that you have authority to bind that organization to these Terms.

2) What HealthBridge provides

HealthBridge supports care coordination by helping you better understand your health needs, benefits, and recommended next steps. The Services may include educational content, reminders, outreach, and tools to support your care journey.

HealthBridge is intended to support—not replace—your clinicians. Always consult qualified providers for medical advice.

3) HIPAA & handling of health information

We treat health information with extra care. When HealthBridge receives, uses, or stores information that is considered protected health information (“PHI”), we do so in a manner intended to align with HIPAA and applicable state privacy laws.

We do not sell PHI. We do not share PHI for third-party marketing. We only share information as needed to support your care, operate the Services, comply with law, or as otherwise permitted by applicable agreements.

  • Care use: to coordinate care and help provide accurate, timely guidance.
  • Operations: to run the Services securely and reliably (with strict controls).
  • Legal requirements: only when required, and only to the minimum extent necessary.

5) Security

We use administrative, technical, and physical safeguards designed to protect personal information and PHI. These may include access controls, audit logging, and encryption in transit and at rest where appropriate.

You are responsible for protecting your account credentials and notifying us if you suspect unauthorized access.

6) Acceptable Use

You agree not to misuse the Services. For example, you will not:

  • Attempt to access data you are not authorized to access;
  • Interfere with or disrupt the Services;
  • Upload malware or attempt to reverse engineer the Services;
  • Use the Services for unlawful, harmful, or deceptive activities.

7) Medical and emergency disclaimer

The Services may provide education, summaries, or suggestions to help you navigate care. The Services do not provide emergency services. If you believe you have a medical emergency, call 911 immediately.

8) Limitation of Liability

To the fullest extent permitted by law, HealthBridge and its affiliates will not be liable for indirect, incidental, special, consequential, or punitive damages arising from your use of the Services.

To the fullest extent permitted by law, HealthBridge’s total liability for claims related to these Terms or the Services will not exceed the amount you paid to HealthBridge for the Services in the twelve (12) months before the event giving rise to the claim, or $100 if you paid nothing.

9) Changes to these Terms

We may update these Terms from time to time. If changes are material, we will take reasonable steps to notify you (for example, by posting a notice or sending a message where appropriate).

Your continued use of the Services after an update becomes effective means you accept the updated Terms.
